The shoot was styled by Katie Grand, and the photography was by Sølve Sundsbø.
Sorry it's heavily tagged, but there are too many ungrateful sites that would happily take these pictures if they were not tagged, and put them on their site without crediting where they got it from.
If they want to share her pictures with their readers they can do what I did, which was buy the magazine, scan it, and upload it.
